New to our family? Great! We are excited for you to become a part of Family Dental Center, where we make smiles happen! We look forward to meeting you!

We strive to make your appointment as easy and comfortable as possible. Please complete the new patient form online prior to your initial visit. This will minimize your time completing paperwork when you arrive at our office, and also allow us to better prepare for your visit.

In order to complete your new patient form, please scroll down and click on “New Patient Profile”. Enter the patient’s first and last name, appointment date, and then hit submit. Select the “New Patient Information & Med Hx Web” form. After you have entered your responses, please hit submit and the information will be sent directly to us.

Once you have been seen at our office, we are happy to assist you with creating your own on-line profile, which is uniquely integrated with your account at our office. Creating your profile will allow you to view your balance, appointments, and also update your account information.

Our goal is to provide the highest quality of dental care possible and to have clear communication about our financial policy.

  • We accept all insurance plans and are participating providers with Delta Dental
  • Cash
  • Check
  • Credit cards including: Discover, MasterCard, Visa

ALL accounts are due and payable at the time of service. If a procedure requires multiple appointments, payment is required in full at the first appointment.

Patients with insurance: The patient IS RESPONSIBLE for the ESTIMATED non-covered portion, procedure and/or deductibles at the time of the service. While we attempt to assist patients as much as possible, the PATIENT is ultimately responsible for understanding their insurance coverage. This includes waiting periods, deductibles and coverage. If you do not understand your policy or coverage, please request this information PRIOR to your appointment.

Parents not accompanying their child to an appointment must make prior arrangements for payment (cash, check or credit card authorization).
